The CDC Dog Import Form is required for every dog entering the United States, regardless of which country it is coming from. It is completed online before travel and links the dog’s microchip number to its travel record and vaccination status. There is no fee.
The Netherlands is not classified as a high-risk country for dog-related rabies, so dogs from the Netherlands do not face the additional USDA facility certification requirements that apply to dogs from higher-risk origins. Your Dutch official vet issues the health certificate, which the NVWA (Netherlands Food and Consumer Product Safety Authority) endorses. Present the CDC form and the endorsed health certificate at the CDC inspection desk on arrival in the USA.

KLM cabin travel across the Atlantic
KLM Royal Dutch Airlines is based at Amsterdam Schiphol and operates direct flights to multiple US cities: New York JFK, Los Angeles, Atlanta, Houston, Boston, Chicago, Miami, San Francisco, Washington DC, and Minneapolis among others. KLM allows small dogs and cats in the passenger cabin, provided the pet plus bag weighs no more than 8 kg and the carrier fits under the seat.
On a transatlantic journey of 8-9 hours, cabin travel is significantly more comfortable for a small pet than the cargo hold. Book the cabin pet space at the same time as your own seat; cabin pet places are limited per flight. The KLM cabin pet fee is typically EUR 75 to EUR 125 each way.
For larger pets that exceed the 8 kg cabin limit, KLM Cargo handles pets in the pressurised hold from Amsterdam Schiphol. The crate must be IATA-compliant and sized appropriately for your pet.
