ISO 11784/11785 microchip required, implanted before the rabies vaccination.
Rabies vaccination

Rabies titre test
Unclear for Iceland specifically, and this matters. MOCCAE exempts roughly 50 named low-risk countries from the titre test. Some pet transport operator sites place Iceland on that list; others do not mention it at all, and MOCCAE's own portal blocks automated verification, so neither claim is fully checkable. Plan on the cautious path: FAVN or RNATT titre test at an accredited laboratory, 0.5 IU/ml minimum, blood drawn at least 30 days after vaccination, then a 90-day wait counted from the draw date, not the result date.
